Symptoms of cirrhosis of the liver
Clinical signs of alcoholic cirrhosis of the liver depend on the degree of damage to liver cells, the activity of the pathological process and associated liver functional insufficiency and portal hypertension.
The earliest signs of alcoholic cirrhosis are weakness, loss of appetite, pain in the right hypochondrium, bloating, alternation of constipation and diarrhea. Pain in the liver area increases after eating, especially fatty foods, and after physical exertion. On examination, dark pigmentation of the skin due to the deposition of melanin is noted. The nails are white and even. Vascular asterisks and palmar erythema are detected in a number of patients. The size and consistency of the liver depend on the stage of the process. At the beginning of the disease, the liver increases in size. With the further development of the process, its consistency becomes denser, while the increase in the liver may be replaced by its decrease. Splenomegaly is detected later than liver enlargement. Along with splenomegaly, other signs of portal hypertension are also determined - varicose veins of the esophagus and hemorrhoidal veins, ascites. The appearance of ascites indicates a developed liver failure. Another complication of cirrhosis of the liver is hemorrhagic syndrome. Patients have massive bleeding from dilated veins of the esophagus and stomach, as well as bleeding from the nose, gums; uterine bleeding, skin hemorrhages are observed.
After massive bleeding, a hepatic coma develops. In the final stage of cirrhosis, cachexia, polyhypovitaminosis develops. The results of laboratory tests in alcoholic cirrhosis of the liver also depend on the degree and activity of the process. In 50-60% of cases, an increase in the level of bilirubin is determined, more often the associated fraction of bilirubin, which increases even with a normal amount of total bilirubin. The development of cirrhosis is characterized by pronounced hypoalbuminemia, hypergammaglobulinemia. Anemia, leukopenia, thrombocytopenia, and increased ESR are observed in more than half of the cases. Violation of the excretory function of the liver in cirrhosis is diagnosed on the basis of a delay in the blood of bromsulphalein and vofaverdin during intravenous administration of them, as well as a decrease in the accumulation of radioactive drugs (gold, bengal pink) during liver scanning.
